GLP-1 Medicines and Fatty Liver (MASH): What the Trials Show

GLP-1 medicines like semaglutide and tirzepatide can cut liver fat and ease MASH. Wegovy is now FDA-approved for MASH with liver scarring. Here is what trials show, the limits, and why weight loss drives the liver benefit.

Fatty liver disease is when fat builds up in the liver. When that fat also brings swelling and liver damage, doctors call it MASH (the new name for NASH). MASH can slowly scar the liver. A group of diabetes and weight medicines called GLP-1 receptor agonists, like semaglutide and tirzepatide, are now being studied and used for MASH because they help people lose weight, and weight loss is one of the best ways to empty fat out of the liver.

What MASH is and why weight loss is the core treatment

NAFLD (now called MASLD) means too much fat in the liver. MASH (the new word for NASH) means fat plus inflammation and liver damage. It is strongly linked to extra body weight, type 2 diabetes, and metabolic problems. The main treatment has long been weight loss, not a pill. Losing even a small amount of weight can pull fat out of the liver, and bigger weight loss can calm the inflammation and scarring. This is exactly why GLP-1 medicines, which are powerful at lowering weight, are now in the spotlight for MASH.

Semaglutide (Wegovy)
Phase 3 ESSENCE trial: MASH resolution without worsening fibrosis was 62.9% with semaglutide vs 34.3% with placebo, and fibrosis improved in 36.8% vs 22.4%. Mean weight loss was 10.5% vs 2.0%. FDA-approved for MASH with F2-F3 fibrosis in August 2025.
Tirzepatide (Zepbound, Mounjaro)
Phase 2 SYNERGY-NASH trial: MASH resolution reached 44-62% across doses vs 10% placebo, and fibrosis improved in 51-55% vs 30%. Strong results, but tirzepatide is NOT yet FDA-approved for MASH. Phase 3 is still running, with results expected around 2027.
Resmetirom (Rezdiffra)
The FIRST drug ever FDA-approved for MASH (March 14, 2024), for noncirrhotic MASH with moderate to advanced fibrosis. It is a thyroid hormone receptor-beta agonist taken as a daily pill, not a GLP-1. It works on liver fat directly.

True, with an important date. The first MASH drug ever approved was resmetirom (Rezdiffra) in March 2024, but it is not a GLP-1. Wegovy (semaglutide) became the first GLP-1 FDA-approved for MASH on August 15, 2025, for adults with noncirrhotic MASH and moderate-to-advanced fibrosis (F2-F3), used with diet and exercise. Tirzepatide (Zepbound, Mounjaro) still has NO MASH approval yet; its strong phase 2 results need phase 3 confirmation.

GLP-1 medicines help MASH mostly by driving real weight loss, which empties fat out of the liver and calms inflammation. The evidence is strong and growing: semaglutide reached about 63% MASH resolution in its phase 3 trial and became the first GLP-1 approved for MASH in August 2025. Tirzepatide looks even better in early phase 2 but is not yet approved for MASH. Resmetirom was the first MASH drug of any kind in March 2024.
